You’ll find all the practical information you need, such as a map and directions to our office(s), practice hours, payment policies and more. We also describe our first visit procedures. And, you can save some time by printing out and completing the new patient forms in advance of your appointment. If you have any questions please call the office at (201) 342.8060.
What to Bring
For conditions such as sinusitis or ear infections, it is helpful to bring the names of medications you have taken for that particular condition. For example, if you have had recurring infections over the past 6 months, try to bring the names of medicines you have taken such as antibiotics, steroids, or nasal sprays. Your pharmacist can quickly print up the list of medicines you have been on (easier to ask for all the medications you have been on for the past 6 months…). If you had a CAT scan or MRI of the sinus or brain, try to bring the report and films as well. If you need assistance, call the office at (201)342-8060.
For thyroid or parathyroid conditions, bring any bloodwork, ultrasound, CAT scan or biopsy results.
